Our online benefits plan is a non-insurance family program, which includes immediate family members and dependents living in the Online Health Benefits Program member’s household. Members and their dependents may not receive benefit savings if they are covered through medical or health insurance, or the service or procedure was started before those persons activated their online health plan membership. Members are responsible for verifying that the chosen Benefit Provider participates in the Health Benefits Program prior to setting up an appointment, by calling MEMBER SERVICES. The Health Benefits Program is a point-of-purchase program whereby members receive member pricing at the time of service and pay for such service when service is received, and whereby member has complied with all Benefits Program requirements including those requirements covered in the Membership Agreement.
